The Clinical Social Worker I provides clinical care to children, adolescents, adults, and families, including responding to the urgent mental health and resource needs of patients seen in outpatient medical clinics. The incumbent will respond to positive suicide screenings and provide crisis-intervention services for urgent psychosocial situations. This position will collaborate on client- and family-centered interdisciplinary teams to link patients with resources and services that will stabilize factors that create barriers to the patient’s health and well-being.

The Clinical Social Worker I is responsible for providing screening, intake assessment and care plan development, case management services, and short-term counseling for families served by the World of Care (WOC) daycare program at PACT. This staff member will facilitate interdisciplinary team meetings, plan/host family engagement activities, and assist with Family Studies internship experiences.
