The Clinical Social Worker I in the Specialized Transition Program provides clinical care to children, adolescents, adults, and families, including mental health and medically-focused psychosocial evaluations, therapeutic interventions, and coordination of care in an intensive day rehabilitation setting. The Psychiatric Clinical Social Worker I provides clinical care to children, adolescents, adults, and families including evaluation, treatment planning, and treatment of mental health conditions. The incumbent provides services utilizing evidence-based/informed treatment models that are best suited to the needs of the patient/family, and coordinates care with multi-disciplinary teams focused on the strengths and effective treatment individualized for the patient and family.
