The Patient Care Coordinator will act as a liaison to patients and families referred to the Nursing Care Managers to arrange appropriate Kennedy Krieger Institute services related to the patient’s special needs in the International Center for Spinal Cord Injury (ICSCI) department. This position requires a high degree of customer service, strong relationship building skills, and cultural sensitivity to manage the needs of clinical staff, patients, and their families.

 

This position is 40 hours per week, working Monday through Friday from 8am-4:30pm (hours are flexible), hybrid.

Responsibilities

  • · Provides intake coordination for referrals specified by the Nursing Care Manager, including sending new patient paperwork. Coordinates and disseminates patient information for review by physicians and Nursing Care Managers.
  • · Obtains and documents patient clinical, demographic, and insurance information in designated Kennedy Krieger Institute forms, databases, and EPIC
  • · Completes and sends medical record requests as necessary.
  • · Communicates with appropriate staff for approval or appropriateness of referral. Follows up with staff and completes all documentation.
  • · Serves as a resource and facilitator to patients and families regarding transportation options, completing necessary forms where appropriate.
  • · Works with families to complete and submit housing referrals.
  • · Maintains an ongoing relationship with the Kennedy Krieger Institute referral liaison and attends outreach meetings as needed.
  • · Produces regular reports regarding Durable Medical Equipment and Transportation forms as requested by the Nurse Case Managers and Director, Nursing Care management.
  • · Reviews Kennedy Krieger Institute EPIC orders for durable medical equipment and processes accordingly.

Qualifications

· High School Diploma or GED preferred.

· Bachelor’s degree in human services or health care administration field preferred.

· Minimum three (3) years of experience coordinating patient services in a medical setting with children and families, preferably those with special needs, required.
