This position is 40 hours per week, working Monday through Friday from 8:00am-4:30pm, hours are flexible and hybrid schedule.

Responsibilities

Responsibilities

Review information provided by the referral source to begin assessment of needs for patients on the inpatient pediatric rehab.
Provide ongoing monitoring of the patient’s progress toward identified goals by reviewing treatment plans, physician recommendations, and/or progress notes.
Develop a plan for teaching of any specific diagnosis or treatment as needed.
Communicate and coordinate teaching with outpatient nursing staff to address identified needs.
Coordinate patient care, serves as liaison, and facilitate communication between family, health care providers, community agencies, and other internal and external customers to ensure care delivery across the continuum.
Participate in the multi-disciplinary team process to facilitate safe and appropriate discharge plans.  
Maintain database of patient information and plans and then synthesizes and analyzes specific data as requested.

 

 

 

Qualifications

 

Education: Graduation from an accredited school of nursing. BSN or enrolled in a BSN program to be completed within 2 ½ years of hiring date.

 

Experience: Minimum (2) years of related experience in a hospital or home care setting required.

 

License: Active, unencumbered Registered Nurse license from Maryland or a compact state.

 

Certification: Current CPR certification that included hands on skills.

 

Related case management, quality assurance, or utilization review certification preferred.  

Proficiency in multi-lingual verbal or written skills a plus; Spanish preferred (must pass a proficiency assessment prior to rendering departmental language support).
