  • Overview

    The Community-Based Programming Coordinator will be responsible for developing and implementing on-going social, wellness, and life skills programming for adults with disabilities. They ensure the quality, health and safety of individuals during community-based programs and at the program site and person-centered approaches for individuals in the community and program site.

    Responsibilities

    1. Implement the Individual Service Plan for each client and provide feedback to the Clinical Services Coordinator.
    2. Assist in the hiring, training and supervision of program assistants.
    3. Assist in creating staff schedules in accordance with client’s individual service plans and Post-Secondary Services program schedule.
    4. Be aware of and submit reportable incidents within required time frames to the Director of Post-Secondary Services.
    5. Work collaboratively with the Post-Secondary Services Team and serve on the admission committee.
    6. Assist in the development, implementation, and monitoring of all Post-Secondary Programs related to social, wellness, and life skills that are community-based and on-site.
    7. Maintain positive relationships with vendors, families, participating individuals, and community partners
    8. Monitor Post-Secondary program registration and develop program registration pages
    9. Monitor and maintain program supplies.

    Qualifications

    EDUCATION:

    High School diploma, or GED, is required.  A Bachelor’s Degree in Special Education, Human Services, or related field is preferred.

     

    EXPERIENCE:

    • Must have at least two years of related experience along with a high school diploma.
    • Must have at least one year of related experience along with an Associate’s degree (or higher).
    • Previous experience with staff supervision and program implementation is preferred.
