    The Social Worker I will provide care coordination services to the patients/families of the Psychiatric Mental Health Program (PMHP). This includes assessment of needs, development of plans, support and assistance in linking to resources and overcoming barriers to access, advocacy and psycho-education. Patients receiving services will be by referral from PMHP clinicians. The Social Worker I may engage in other social work roles within the scope of practice for a Licensed Bachelor Social Worker (LBSW) as assigned.

    Responsibilities

    1. Accept referrals for care coordination services according to the process designated by the care coordination manager.
    2. Utilize protocols for gathering information, assessing needs, maintaining a record of contacts, and providing a record of case activities and status through referral database.
    3. Will complete and submit documentation--for supervisor approval--for the medical record in a timely manner.
    4. Will adhere to standards of care and continuing education outlined in the Case Management chapter 10.42.02 Code of Maryland Regulations.
    5. Establish and maintain contact with the referring clinician and the patient and family while cases are open.
    6. May provide resources to the BSW interns.
    7. Will collaborate with administrative staff in updating and maintaining resource and referral databases.
    8. Attend and participate in staff meetings and trainings.
    9. Participate on committees and workgroups within the PMHP as assigned.
    10. Participate in implementation of interventions (e.g. group sessions) with approval of the supervisor, and in collaboration with the facilitating clinician.

    Qualifications

    EDUCATION:

    • A bachelor degree in Social Work from an accredited college or university is required.

    EXPERIENCE:

    • Must have at least one year’s experience in providing care coordination services ( may include licensed, non-licensed and/or internship experience).
    • Experience in a medical, or mental health setting is preferred.
