    The Clinical Services Coordinator will be responsible for the documentation of individual client programs and building relationships with the community and employers. The Clinical Services Coordinator will deal effectively and professionally with government agencies, the general public, individuals with disabilities and their families.

     

     

    Responsibilities

    1. Monitor and manage documentation of individual client program development and implementation; document program updates as needed.
    2. Maintain and update policies and procedures which ensure compliance with COMAR regulations.
    3. Maintain case management binders, document communications between clients and their families, and ensure proper documents are submitted to the appropriate parties.   
    4. Coordinate the documentation of the assessment, development, and implementation of Service Funding Plans (SFP) and Individual Plans (IP). May also administer assessments, as needed.
    5. Observe potential clients as part of the admissions process to help assess skills and needs, and may observe existing clients to monitor progress.
    6. Prepare Service Plans for the admissions team and participate in the client admissions process.
    7. Assist in developing and maintaining relationships with State agencies including the Department of Health & Mental Hygiene (DMHM), the Developmental Disabilities Administration (DDA); Maryland State Department of Education (MSDE); and the Department of Rehabilitative Services (DORS).
    8. Assist in the administrative preparation and maintenance of all documentation to obtain and sustain DDA provider status.
    9. Maintain information in organized files and produce needed documentation upon request. Ensure that proper client documentation is maintained and updated.
    10. Lead and/or participate in Person-Centered Planning meetings.
    11. Prepare billing documentation of client services for state agencies.
    12. Maintain regular contact and facilitate ongoing partnerships with all community-based employers and companies providing contract work, and actively work to develop personalized community-based and/or center-based employment options for program participants.

     

    Qualifications

    EDUCATION:

    Bachelor’s Degree in Special Education or related field is preferred.

     

    EXPERIENCE:

    Minimum of three years of experience in special education or vocational rehabilitation required.
