The Nurse Coordinator  participates in evaluation of the  client’s  physical status at referral, at intake, within 30 days of placement, and minimally quarterly thereafter.  The Nurse Coordinator provides training, education, and consultation for Therapeutic Foster Care Clinical Social Workers, treatment parents, clients, and their families/caregivers regarding medication, diagnosis, medical conditions, developmental disabilities, and availability of appropriate resources.  The Nurse Coordinator acts as a liaison for clients and families assessing and identifying issues and needs to be reviewed by the KKI Director of Outpatient Services, medical specialist and primary care physician.  The Nurse Coordinator helps promote continuity of care between the various medical specialists, clinical social worker, the Department of Social Services, the patient/family, and other appropriate outside agencies. 







  1. Provides direct nursing care including, but not limited to obtaining vital signs, performing assessments, reviewing for medication side effects, and communicates information obtained to other medical professionals and treatment team members.
  2. Participates as a member of the TFC treatment team as evidenced by meeting with client during treatment in the parent’s home at placement, 30 days following placement, and minimally quarterly thereafter, which includes participation in the development of the client’s individual service or treatment plan.
  3. Assists the Intake Coordinator in reviewing referrals and coordinating placements with appropriate treatment parents, which includes coordination of necessary information with external medical professionals, the Director of KKI Outpatient Services, and Nurse Manager of Outpatient Services and School-Based Programs to address potential medical needs and areas of risk.
  4. Assists in the coordination of services for clients discharged from the program and ensures availability of necessary medical follow up with family or other discharge placement.
  5. Provides orientation, training, and consultation for Clinical Social Workers, treatment parents, Department of Social Services, client, family, and other treatment team members, which includes the coordination of training and consultation with appropriate KKI and outside medical professionals.
  6. Prepares and obtains educational resources and material to be utilized in the program. Acts as resource for TFC Clinical Social Workers, treatment parents, clients and families in identifying community resources consistent with the client’s needs.
  7. Assists clinicians in triaging emergencies and provides follow up with medical professionals, treatment team members, and when appropriate the Nurse Manager of Outpatient Services and School-Based Programs, the Director of KKI Regulatory Affairs, and the DHR Office of Licensing and Monitoring
  8. Regularly reviews individual client’s risk plans with the TFC Program Director, which includes providing recommendations for improvements in the overall TFC Program risk management plan
  9. Obtains and records specific information in department wide database. Documents all child/youth/family and collateral contacts in the program’s database.
  10. Assists in communicating client’s needs to outside agencies through multiple medium (letters, phone, fax, E-mail) as needed in accordance with patient rights and confidentiality policy.
  11. Assists in the collection of data for use in the program performance evaluation and performance improvement plans as well as other outcome or research projects.
  12. Prepares externals workshop or conference presentations and publications.



  • Current Registered Nurse Licensure in the State of Maryland
  • Graduate of an accredited school of nursing with a Bachelor os Science in Nursing preferred.


  • Organizational, problem-solving, and good communication skills are required.
